Job summary

Community Healthcare of Texas is seeking an RN Admission Nurse in the Dallas area to meet with patients and families, explain the scope of hospice services, and coordinate patient admissions. The role includes conducting thorough admission assessments, arranging initial nursing visits and DME/RX needs, and collaborating with the Interdisciplinary Team to ensure comprehensive patient care.

Qualified applicants are Texas-licensed RNs with at least 1 year of nursing experience; hospice case

Qualifications

• Must be a Texas-licensed Registered Nurse.
• Minimum of 1 year of nursing experience required.
• 2+ years hospice case management preferred.
• Must have a valid driver’s license.
• Proficient in Microsoft Office Suite.

Responsibilities

• Meet with patients and families to explain hospice services and coordinate admission.
• Complete thorough admission assessments and arrange initial nursing visits and DME/RX needs.
• Attend and report to the Interdisciplinary Team regarding the patient.
• Complete paperwork and charting in accordance with policy.
• May conduct nursing or 'tuck-in' visits and train admission staff.
