Job summary

Compassus in Texas is seeking a Hospice Admission Registered Nurse (RN) to join a compassionate team delivering home-based hospice care. You will assess patients for hospice eligibility, coordinate initial care plans, and support families through an often difficult journey.

Qualifications

RN license in Texas; hospice experience preferred.
Current CPR and BCLS certification.
Valid driver’s license required.

Responsibilities

Enable patients to spend quality time with their loved ones and doing activities they enjoy.
Assess patients for hospice eligibility and explain hospice care services to patients, families/caregivers.
Ensure all patient’s initial needs are addressed.
Collaborate with the Inter-Disciplinary Team members regarding initial plan of care, patient’s condition, and other essential information.
