As the first trusted connection in hospice admissions, you will assess needs, develop care plans, educate families, coordinate services with physicians and the care team, and maintain accurate documentation while balancing travel

Qualifications

• Active Texas RN license required.
• Minimum 1 year nursing experience; hospice or home health a plus.
• Excellent communication and interpersonal skills.
• Strong organizational and critical thinking abilities.
• Ability to work independently and within a team; willingness to travel.
• Physical ability to lift up to 55 pounds.

Responsibilities

• Lead the admissions process with compassion and professionalism.
