Job summary

Desert Valley Hospice & Palliative Care in Arizona seeks a Hospice Admission Nurse to evaluate and admit patients to hospice care, assess medical conditions, and clearly explain services to patients and families.

You will collaborate with physicians, social workers, and other clinicians to develop individualized care plans, ensure all paperwork is completed, and provide comfort-focused, compassionate care during difficult transitions.

Qualifications

• Current, active RN license in Arizona with fingerprint clearance.
• Current CPR certification.
• At least 1 year of hospice RN experience.

Responsibilities

• Evaluate and admit patients to hospice care.
• Assess medical condition and explain services to patients and families.
• Collaborate with providers, social workers, and other healthcare professionals to create care plans.
• Ensure all paperwork is completed.
• Provide comfort-focused and compassionate care during transitions.
