Affinity Health Management is seeking an Admission RN to plan, organize, and direct hospice care in home and community settings in New York. The role emphasizes nursing expertise with a focus on community health education and coordination of comprehensive patient care.

The RN will assess patients and families, develop care plans, administer treatments, support daily living activities, and collaborate with the interdisciplinary team to ensure high-quality end-of-life care.

Qualifications

• RN license required.
• Hospice/end-of-life care experience preferred.
• Strong assessment and care-planning skills.
• Excellent communication with patients, families and healthcare team.

Responsabilités

• Conduct initial, comprehensive patient and family assessment to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Develop and update care plans with patient/family involvement.
• Initiate and revise plans of care based on patient status and changing needs.
• Administer medications and treatments as prescribed and monitor outcomes.
• Counsel patients and families on nursing needs and self-care.
• Educate patients and families about health management and comfort measures.
• Ensure safe home environments and coordinate interdisciplinary care.
• Coordinate patient care as Case Manager when assigned by Clinical Supervisor.
• Keep accurate clinical notes and report changes to physicians.
