Job summary

Iowa City Hospice in Iowa City is seeking an Admissions RN to create a compassionate start for patients entering hospice. You will assess needs, determine eligibility, and coordinate with the care team for timely admissions while aligning with hospital and facility referral activity.

Join a mission-driven team offering education, strong collaboration, and a focus on dignity and comfort at end of life, with a schedule Monday–Friday 10:00 AM–6:00 PM and after-hours call rotation.

Qualifications

• Current Iowa RN license in good standing.
• Graduate of an approved school of nursing (RN/BSN/ARNP) with a current Iowa Nursing License.
• Two years of home health or hospice experience preferred.
• Advanced Certifications in Hospice and Palliative Nursing by year three.

Responsibilities

• Respond to hospice referrals and inquiries promptly.
• Conduct comprehensive admissions assessments in homes and facilities.
• Evaluate hospice eligibility per regulatory and clinical guidelines.
• Explain hospice services, philosophy of care, and patient rights.
• Coordinate with physicians, social workers, and care teams for timely admissions.
• Complete admission paperwork accurately and compliantly.
• Develop initial plans of care addressing patient and family needs.
• Provide education on disease progression, symptom management, and support services.
• Maintain accurate documentation in the EMR and participate in on-call coverage.
