Job summary

Iowa City Hospice is seeking an Admissions RN to create a compassionate, trusted hospice experience from the start. This full-time role follows a 10:00 AM–6:00 PM Monday–Friday schedule and participates in after-hours and holiday call rotations to ensure timely access to end-of-life care.

Responsibilities include prompt referrals response, comprehensive home/hospital admissions assessments, and collaboration with physicians, social workers, and care teams to admit patients with sensitivity and

Qualifications

• Current Iowa RN license in good standing.
• Graduate of an approved nursing program with active Iowa license.
• Two years of home health or hospice experience preferred.
• Valid driver’s license and reliable transportation.

Responsibilities

• Respond promptly to hospice referrals and inquiries.
• Conduct comprehensive admissions assessments in homes, hospitals, and facilities.
• Evaluate patients for hospice eligibility per guidelines.
• Explain hospice services and patient rights to families.
• Collaborate with physicians, social workers, and care teams to admit patients.
• Complete admission paperwork accurately and compliantly.
• Coordinate initiation of services with the hospice team.
• Develop initial plans of care addressing patient and family needs.
• Provide education on disease progression, symptoms, and care options.
• Maintain accurate EMR documentation.
• Participate in on-call or weekend admissions as scheduled.
• Represent Iowa City Hospice professionally in the community.
