Job summary

WesleyLife in Urbandale, IA is seeking an Admissions Nurse for our Hospice team to travel to CHLs and patient homes, performing comprehensive assessments and initiating a plan of care. You will drive patient-centered care across settings and coordinate with clinical teams.

Qualifications

• Active Iowa RN license is required.
• Must have a valid Driver's License.
• Current RN registration in good standing.

Responsibilities

• Travel to CHLs and homes to assess hospice patients and families.
• Perform initial, resumption, follow-up, and discharge assessments including HIS and plan of care.
• Initiate and update individualized care plans for changes in status and re-certifications.
• Use health data to determine nursing diagnoses and care needs.
• Obtain and implement orders from physicians overseeing patient care.
• Administer medications and treatments as prescribed.
• Counsel patients and families on nursing needs and self-care.
• Provide health instructions per assessment and plan.
• Assist patients with activities of daily living and support comfort care when appropriate.

Job description

As an Admissions Nurse for our Hospice team, you will:

• Traveling to CHLs and homes to see our hospice clients and their families
• Complete initial, resumption, follow up, a significant change of status and discharge assessments including comprehensive head to toe assessment, HIS assessment, and all associated assessments with admission or discharge
• Initiate an individualized plan of care, and update the plan of care for resumptions, significant change in status, and re-certifications.
• Use health assessment data to determine nursing diagnosis
• Obtain, implement, perform and maintain orders as received by physicians overseeing patient care services
• Initiate appropriate preventive and rehabilitative nursing procedures. Administer medications and treatments as prescribed by the physician
• Counsel the patient and family in meeting nursing and related needs
• Provide health care instructions to the patient as appropriate per assessment and plan of care
• Assist the patient with activities of daily living and facilitates the patient’s efforts towards self-sufficiency and optional comfort care.
