Job summary
Trustbridge Hospice, part of Empath Health, is seeking a compassionate Registered Nurse to join the Admissions Team serving Palm Beach County. The RN will evaluate referred patients, educate families about hospice care, and coordinate individualized plans of care in collaboration with physicians and liaisons.
Schedule is 11am - 9:30pm with every other weekend shift. The role requires a Florida RN license, BLS, hospice or related experience, and a valid Florida driver’s license.
Qualifications
- Active Florida RN license.
- BSN preferred; Hospice certification preferred.
- Minimum of one year in public health, home health, oncology, medical/surgical, geriatric, or hospice nursing.
- Ability to assess physical and psychosocial needs and communicate clearly.
- Valid Florida driver’s license and reliable transportation; EMR experience a plus.
Responsibilities
- Serve as the primary nurse evaluating patients referred for hospice services.
- Educate patients and families on hospice care and the continuum of care.
- Coordinate individualized plans of care with physicians and liaisons.
- Act as liaison between Trustbridge Hospice and hospitals, long-term care facilities, and other providers.
