Qualifications

• Must meet state regulatory requirements and be able to work on-call.

Responsibilities

• Assess patient and family needs and collaborate to establish the plan of care.
• Coordinate with physician and IDG to optimize care and resources.
• Document per protocol and maintain up-to-date records.
• Participate in on-call or admissions rotation as directed.
• Educate patients and caregivers according to the plan of care.
