Job summary

Peoples Hospice is seeking a Hospice Admissions Nurse to perform hospice consultations and admission visits, collaborate with physicians, and help determine patient eligibility for hospice care. You will manage a caseload of four to five patients and develop plans of care with the IDG.

This role requires an active Florida (or multi-state) RN license, CPR cert, and the ability to work evenings, nights, weekends, and holidays. Local travel and healthcare teamwork are essential.

Qualifications

• Current Florida or multi-state RN license is required.
• Two years of RN experience is preferred.
• Experience in acute, oncology, home health, and/or hospice setting preferred.
• Availability to work evenings, nights, weekends, and holidays.
• Current CPR certification is required.
• Valid driver’s license and current auto insurance is required.

Responsibilities

• Performs hospice consultation and admission visits to assess patient/family needs and determine clinical eligibility.
• Completes admission assessment and Hospice Election of Benefit documentation.
• Maintains accurate records and submits required reports on time.
• Collaborates with physicians and IDG to coordinate patient care and services.
• Manages a caseload of four to five patients and develops/update Plans of Care.
