Qualifications

• Licensed RN in practicing state; associate degree preferred; CPR recommended.
• Must meet regulatory requirements for hospice/home health roles.
• Experience in hospice, home health, or general medical-surgical settings.
• Willingness to participate in on-call rotations.

Responsibilities

• Assess patient and family needs and establish care plans with IDG.
• Coordinate admissions with physicians and care teams, ensuring timely start of care.
• Communicate with attending physicians, Medical Director, and IDG.
• Document per protocol and maintain accurate patient records.
• Participate in on-call or admissions rotations as directed.
• Educate patients and caregivers about plan of care and expectations.
