GBMC HealthCare's Hospice Admissions Nurse role seeks a compassionate RN to be the first point of contact for patients and families entering hospice. You will complete admissions, educate families, coordinate with the interdisciplinary team, and help ensure a smooth transition into hospice services.

Qualifications

• Diploma, ADN, and/or BSN from an accredited institution.
• Three years of nursing; Home Health or Hospice experience preferred.
• Current Virginia state license as a Registered Nurse.
• Valid Driver’s License and insured vehicle.

Responsibilities

• Completes initial patient and family assessments for hospice eligibility and needs.
• Prepares and implements the initial care plan with Medical Director approval.
• Provides direct nursing care to maintain high quality.
• Informs management of resource utilization and referrals.
• Documents clinical findings and participates in discharge planning with care teams.
• Educates family and caregivers on pain control, medications, and care skills.
• Arranges home equipment as needed.
• Participates in admissions projects and process improvements.
