Job summary

Gilchrist Inova is seeking a compassionate Hospice Admissions Nurse to assess patients in their homes or hospitals and prepare them for entry into hospice services. You will serve as the first point of contact and ambassador for Gilchrist Inova in the service area, coordinating with the interdisciplinary team.

The role requires an RN with strong clinical and communication skills, at least three years of nursing experience, and a Virginia license.

Qualifications

• Current Virginia RN license.
• Three years of nursing experience.
• Home Health or Hospice experience preferred.
• Knowledge of infection control and safety practices.

Responsibilities

• Completes initial assessment of patient and family to determine hospice eligibility and needs.
• Prepares and implements the initial care plan with Medical Director approval.
• Provides direct nursing care to maintain high-quality patient care.
• Coordinates with the interdisciplinary care team and other departments.
• Educates family members on pain control, medications, and caregiving tasks.
• Documents findings in the EMR and participates in discharge planning when needed.
