Qualifications

• Current MD nursing license.
• Three years of nursing experience. Home Health or Hospice experience preferred but not required.

Responsibilities

• Completes initial assessment of patient and family/caregivers to ascertain Hospice eligibility.
• Prepares and implements the initial care plan for patient/family unit.
• Provides direct nursing care to patients.
• Informs Team Leader of utilization of other team members and resources.
• Documents clinical findings and participates in discharge planning.

Skills

Knowledgeable of universal precautions for infection control and practices safe nursing standards for self protection

Strong time management skills

Excellent interpersonal and communication skills

Ability to be patient and family focused and provide patient/caregiver education

Skill in the use of EMR (Electronic Medical Record)

Education

Diploma, ADN and/or BSN from an accredited institution
