Residential Home Health and Residential Hospice is hiring an Admission RN for hospice care in the Lewisburg, PA area. You will conduct comprehensive admissions assessments, reconcile meds, and develop individualized care plans in collaboration with physicians.

Travel to patient homes is required (about 20% of the time). The role offers meaningful patient relationships, autonomy, and a $10,000 first-year bonus; a strong leadership and education team supports your growth.

Qualifications

• Active RN license in the state you work.
• Two years of RN experience; hospice experience preferred.
• Valid driver's license required for field visits.

Responsibilities

• Deliver compassionate, patient-centered care in a hospice setting, with an interdisciplinary team.
• Conduct comprehensive hospice admission assessments and develop individualized plans of care.
• Educate patients and families through the admission process and support transition to end-of-life care.
• Apply clinical judgment to facilitate timely admissions and accurate EMR documentation.
• Coordinate evaluations to other disciplines when indicated.
• Travel to patient homes within assigned territory and work independently but within a team.

We are hiring an Admission Registered Nurse (RN) for a Hospice position providing end-of-life care to patients in their homes in the Lewisburg, PA area. This position is eligible for a $ 10,000 first year success bonus to reward the accomplishment of key achievements during your first year!

What You'll Do:

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life
• Conduct comprehensive hospice admission assessments, including clinical evaluations, medication reconciliation, and development of individualized plans of care aligned with physician orders
• Educate and guide patients and families through the hospice admission process, setting expectations, explaining the hospice philosophy, and ensuring they feel informed, supported, and comfortable with the transition to end-of-life care
• Apply strong clinical judgment and compassionate critical thinking to facilitate timely admissions, address immediate needs, and ensure accurate, thorough, and compliant EMR documentation
• Initiate evaluations to other disciplines when indicated
• Work independently, but never alone
• Travel to patient homes within assigned territory

Admission RN Qualifications

• Active Registered Nurse, RN license in the state you work

• Two years of RN experience and the clinical expertise to deliver quality patient care, hospice experience preferred

• Due to the field-based nature of this home health position, a valid driver's license and the ability to spend approximately 20% of your workday driving to and from patient locations are required
