Chapters Health System in the DC area is seeking an Admissions Nurse, RN to coordinate hospice admissions and collaborate with an interdisciplinary team to deliver compassionate patient care.

Requirements include an active RN license in the applicable state and at least 1 year of nursing experience, with hospice or hospital background preferred; EMR/EHR experience is required, English proficiency is essential, and bi-lingual in Spanish is a plus.

• Experience with an EMR/EHR system.

Responsibilities

• Complete clinical admissions for all company services and explain services to patients/families.
• Initiate plan of care and nursing interventions to address needs and prevent complications.
• Coordinate with physicians, facilities, and care navigation for timely follow-up and referrals.
