Job summary

Interim HealthCare Hospice is hiring a Hospice Admissions RN to provide comfort, strength and guidance to patients and families at life’s final chapter. Conduct initial home or facility assessments, start the plan of care, and coordinate admissions with the IDT and physicians.

The role requires an Indiana RN license and at least 1 year of nursing experience, preferably in hospice or home health care, with CPR certification.

Qualifications

• Active RN license in Indiana is required.
• Minimum 1 year of nursing experience, ideally in hospice or home health care.
• CPR certification is required.

Responsibilities

• Conduct timely initial hospice assessments in patients' homes or facilities, documenting physical, psychosocial, and medication histories to determine eligibility and care needs.
• Initiate the interdisciplinary plan of care by writing nursing diagnoses, setting palliative goals, and obtaining physician orders and consent forms at admission.
• Coordinate referrals, verify insurance, and ensure smooth admissions as the clinical liaison among patients, families, physicians, and IDT.
• Provide start-of-care nursing interventions, symptom management teaching, and caregiver education to optimize comfort and safety.
• Document all assessments, care plans, and communications in the EMR in compliance with Medicare/state regulations.
• Educate family on prognosis, changes in care plan, medications and hospice goals.
• Take a holistic approach engaging the patient’s mind, body, spirit and family.
