Job summary

Heart to Heart Hospice is seeking an Admissions RN to join our team. You will conduct initial patient and family assessments, explain hospice services, and coordinate with the care team to develop and implement care plans.

The role requires nursing licensure and a valid driver’s license, with 1–3 years of RN experience in clinical settings, hospice, or home health preferred. We offer full-time benefits and a supportive, compassionate work environment.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State Registered Nurse License.
• Current Driver's License, reliable transportation, and provide ongoing valid auto liability insurance.
• 1–3 years of RN experience in clinical care; hospice or home health preferred.

Responsibilities

• Demonstrate autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.
• Review clinical referral information and respond to requests and inquiries as appropriate.
• Collaborate with Patient Care Manager, attending physician and hospice physician regarding eligibility and appropriateness of client for hospice services.
• Conduct initial meeting with patient and family about hospice services and obtain care consents.
• Provide initial comprehensive assessment of patient and family needs (physical, psychological, social, spiritual) and initiate interventions.
• Develop initial care plan with goals and interventions based on nursing assessment and team input.
• Maintain regular communication with Intake Coordinator, Patient Care Managers, RN Case Manager and core team to review patients and needs.
• Timely submission of all documentation.
• Perform other duties as required and ensure compliance with laws, regulations, and policies.
