Job summary

Residential Home Health and Residential Hospice is seeking an Admission RN to deliver end-of-life care in patients' homes in the Northbrook, IL area. You will assess needs, coordinate with an interdisciplinary team, and guide families through the admission process with compassion.

This role requires an active IL RN license and at least two years of RN experience (hospice preferred). A valid driver's license is required, and travel to patient locations is expected.

Qualifications

• Active RN license in the state you work.
• Two years of RN experience; hospice experience preferred.
• Valid driver’s license and ability to drive ~20% of the day

Responsibilities

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life
• Conduct comprehensive hospice admission assessments, including clinical evaluations, medication reconciliation, and development of individualized plans of care aligned with physician orders
• Educate and guide patients and families through the hospice admission process, setting expectations, explaining the hospice philosophy, and ensuring they feel informed, supported, and comfortable with the transition to end-of-life care
• Apply strong clinical judgment and compassionate critical thinking to facilitate timely admissions, address immediate needs, and ensure accurate, thorough, and compliant EMR documentation
• Initiate evaluations to other disciplines when indicated
• Work independently, but never alone
• Travel to patient homes within assigned territory

Admission RN Qualifications

• Active Registered Nurse, RN license in the state you work
• Two years of RN experience and the clinical expertise to deliver quality patient care, hospice experience preferred
• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required
