Vitality Hospice is seeking an Admissions Registered Nurse (RN) to join our clinician-owned hospice agency. The role focuses on comprehensive hospice admissions, patient/family assessments, and creating individualized care plans with the interdisciplinary team.

Qualifications

• Active RN license is required.
• 1+ year of skilled nursing experience preferred.
• Hospice or admissions experience preferred (not required).
• Willingness to travel within assigned service areas.
• Strong communication, organizational, and interpersonal skills.
• Valid driver’s license, reliable transportation, and auto liability insurance.

Responsibilities

• Complete comprehensive hospice admissions and patient/family assessments.
• Ensure accurate documentation and appropriate care planning.
• Provide direct patient care as needed, including medication administration and treatments.
• Collaborate with physicians and the interdisciplinary team to develop personalized plans of care.
• Serve as a liaison between patients, families, and healthcare providers.
• Educate patients and caregivers on disease progression, symptom management, and hospice services.
