WesleyLife seeks an Admissions Nurse for its Hospice team to travel to client homes and facilities, conducting comprehensive assessments and initiating individualized care plans. The role involves coordinating orders, providing patient and family education, and supporting daily living needs in the home care setting.

The nurse will perform head-to-toe exams, HIS assessments, and adjust plans as patient status changes, working closely with physicians and caregivers to ensure high-quality hospice

Qualifications

• Initial and follow-up nursing assessments at clients' homes or facilities.
• Administer medications and implement physician orders.
• Develop and update individualized plan of care.
• Counsel patients and families on nursing and care needs.
• Travel to CHLs and homes to visit hospice clients.

Responsabilités

• Perform head-to-toe assessments and HIS assessments.
• Initiate and revise the care plan for resumptions or changes in status.
• Obtain and execute orders from physicians overseeing patient care.
• Provide health instructions and caregiver support.
• Assist with activities of daily living and comfort-focused care as appropriate.
