Job summary

Gilchrist Inova Hospice Care seeks a Hospice Liaison Nurse to generate and facilitate patient admissions from hospital-based referral sources. You will promote growth of admissions and coordinate clinical assessments of prospective patients in a fast-paced field setting across Northern VA.

You will establish and nurture external professional relationships, represent Gilchrist Inova professionally, and maintain HIPAA and regulatory compliance while supporting the Admissions & Business Development

Qualifications

• Must have RN license in Virginia.
• Associate degree or Nursing Diploma required.
• Experience in hospice, home health, critical care, ER or similar areas preferred.

Responsibilities

• Generate and facilitate patient admissions from hospital-based referrals.
• Build and maintain relationships with area hospitals and referral sources.
• Perform or coordinate clinical assessments of prospective patients.
• Maintain HIPAA/privacy and regulatory compliance.
• Document and communicate patient status and admissions effectively.
