Qualifications

• BSN preferred and CT RN license required.
• Experience in home health or acute care setting.
• Knowledge of Medicare/Medicaid regulations and OASIS.

Responsibilities

• Admit patients to hospice and palliative care services with initial plan of care.
• Complete HIS and comprehensive assessment accurately and timely.
• Document patient information and OASIS data set in the system.
• Develop patient-specific care plans addressing active problems and goals.
• Coordinate medication reconciliation and patient teaching.
• Identify payer qualifications and ensure correct coverage notifications.
• Communicate effectively with patients and families of diverse backgrounds.
