Qualifications

• RN licensed in Connecticut with a current license.
• BSN preferred.
• Knowledge of Medicare/Medicaid regulations.
• Experience in home health or acute care setting.

Responsibilities

• Admit patients to hospice service and develop initial plan of care.
• Perform holistic assessment of patient and family needs.
• Complete HIS data set and OASIS accompanying documentation.
• Document patient information accurately in computer systems.
• Coordinate care with patient and family to establish measurable goals.
• Identify payer for services and obtain necessary authorizations.
