Job summary

Three Oaks Hospice, Inc. seeks a registered nurse to provide skilled nursing care in patients’ homes within the Temple area. The RN coordinates care with the interdisciplinary team (IDT), educates patients and families, and ensures adherence to hospice protocols and end-of-life care plans.

The role requires an RN license in Texas, CPR certification, and at least two years of clinical nursing experience, with hospice or home health exposure preferred. On-call duties may apply.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state RN license and driver’s license.
• Two years RN experience in a clinical care setting; hospice/home health preferred.
• Current CPR certification.
• Proficient with computers; MS Office experience preferred.
• Strong interpersonal, communication, and organizational skills.
• Ability to cope with family emotional stress.

Responsibilities

• Perform initial and ongoing comprehensive assessments of patient needs and ADLs.
• Implement and revise the care plan as needed.
• Educate patient/family on disease, self-care, end-of-life care, nutrition.
• Provide training to other staff as needed.
• Initiate preventive and rehabilitative nursing procedures.
• Prepare clinical and progress notes showing progress toward goals.
• Coordinate all patient/family services with the IDT and referrals.
• Maintain on-call responsibilities and provide on-call care.
• Communicate information using current processes and technology.
