Job summary

Visiting Nurse Health System in Atlanta is seeking an Admissions Nurse for Hospice to coordinate care for a home-based caseload and ensure quality standards. You will assess, plan, implement, monitor, and evaluate comprehensive care plans to optimize patient outcomes.

The role requires leading patient education, collaborating with an interdisciplinary team, and supervising LPNs/HHAs while managing on-call duties. Georgia licensure and CPR are required.

Qualifications

• Graduate from an accredited nursing program with valid Georgia RN license.
• Current CPR certification and Georgia State Board licensure.
• Home health experience preferred; hospice experience a plus.
• Ability to collaborate with interdisciplinary teams and educate patients.

Responsibilities

• Coordinate care for hospice patients; ensure quality care plans.
• Assess, plan, implement, monitor and evaluate patient care at home.
• Complete OASIS documents and home health records timely.
• Educate patients and families on disease management and self-care.
• Supervise LPNs and HHAs per VNHS policy.
