Qualifications

• High school diploma or GED required.
• Bachelor’s degree preferred.
• 2–3 years of related experience strongly preferred. An equivalent combination of education and experience will be considered.
• Experience communicating with physicians, nurses and other healthcare personnel highly preferred.
• Experience in insurance and Medicare/Medicaid billing and reimbursement preferred.

Responsibilities

• Process patient referrals and coordinate admissions.
• Enter admission data into the system after all requirements are met.
• Coordinate with physicians, insurance and care teams to facilitate admissions.
• Schedule admissions appointments and obtain informed consents.
• Explain hospice services and benefits to patients and families.
• Maintain accurate patient charts and communicate updates to the IDT.
