Job summary

Compassus in Bedford, NH is seeking a Hospice Admissions Coordinator to process referrals, coordinate patient admissions, and enter data after all hospice requirements are met.

You will work with physicians, payors, and care teams to obtain orders, arrange pre-certifications, schedule assessments, and explain hospice services to families; ensure accurate charting and timely handoffs to the IDT. This on-site role emphasizes compassionate communication and meticulous documentation.

Qualifications

• High school diploma or GED is required.
• Bachelor's degree preferred.
• 2–3 years of related experience and/or training strongly preferred.
• Equivalent education/experience will be considered.
• Experience communicating with physicians, nurses and other healthcare personnel is highly preferred.
• Experience in insurance and Medicare/Medicaid billing and reimbursement preferred.

Responsibilities

• Support designated program area and/or region.
• Attend and participate in staff meetings, educational programs and community events as requested.
• Communicate identified needs and potential solutions to supervisor.
• Receive and process referrals from physicians, patients, families, friends, and community.
• Contact physicians to secure an order for admission.
• If Admissions Coordinator is not an RN, ensure a nurse is present or obtain a script to fax.
• Contact insurance companies for pre-certification authorization.
• Schedule appointments for informed consents and RN assessment visits.
• Explain hospice services and benefits to patients, families, and others as needed.
• Inform team members of appointment times.
• Complete admission process according to policy.
• Notify IDT and ancillary providers of admissions.
• Enter admission data into a third‑party system.
• Ensure patient chart is signed/dated and turned over to Team Coordinator.
• Market referral sources.
• Contact long term care staff, hospital staff, and physicians to promote hospice services.
• Provide back-up support for Hospice Care Consultant.
• Document marketing contacts as directed.
• Perform other duties as assigned.
