Job summary

Chapters Health System is seeking an Admissions Facility Coordinator to facilitate hospice admissions in long-term care facilities under the Admissions Manager. The role focuses on building and maintaining relationships with referral sources, coordinating admissions, and ensuring timely processing and documentation in the EMR.

You will collaborate with procurement and facility staff, promote solutions for billing questions, and support ongoing customer service and patient transfers.

Qualifications

• High school diploma or equivalent is required.
• Minimum two years of customer service experience.
• Basic computer skills are required.
• Good interpersonal and communication skills are essential.
• Established positive relationships within long-term care communities are preferred.
• Valid driver’s license and auto liability insurance in the state of care are required.

Responsibilities

• Develops, expands, and maintains quality professional relationships with referral sources in LTC/ALF facilities.
• Maintains a consistent professional presence in the territory to meet admission goals.
• Receives hospice referrals and discusses goals of care with patients/families.
• Provides information to referral sources, including billing updates and questions.
• Identifies admission barriers and discusses them with involved parties.
• Maintains regular communication with facilities to enhance relationships.
• Explains hospice services to patients/families and gathers necessary admission information.
• Discusses financial obligations and insurance coverage with patients/families.
• Enters accurate encounter information into the EMR related to insurance, physicians, demographics, etc.
• Completes documentation within the EMR at time of care and by end of shift.
• Mentors liaisons and clinical staff with regular feedback.
• Interacts with facility referral sources and provides feedback.
• Communicates immediate care needs to Admissions Manager or designee.
• Maintains positive relations with other departments and responds promptly to requests.
• Performs other related duties as assigned.
