Job summary

AccentCare is seeking a Director of Admissions, Hospice in Clearwater, FL. This on-site role leads the admissions team, coordinates referrals, and ensures regulatory compliance while driving care coordination from inquiry to admission.

The candidate will oversee staff, mentor teams, and partner with physicians and other providers to optimize patient care. Competitive benefits and PTO accompany the position.

Qualifications

• Bachelor’s degree required.
• 4+ years hospice/home health or case management experience.
• RN licensure in Florida and valid driver’s license.

Responsibilities

• Develops, coaches and serves as a resource and liaison to the Admissions Team and all staff who assist in the process from referral inquiry to admission.
• Ensure all patient and family services offered by the Hospice, during the referral inquiry to admission process, comply with federal, state(s) and local regulatory requirements.
• Communicate with attending physicians, other hospice staff members and other agencies as needed to coordinate optimal care and use of resources for the patient and families.
• Evaluate staff productivity, referral and admission data to ensure adequate staffing and meet budgetary requirements.
• Mentor and model follow-up activities on active and pending referrals to meet conversion rate goals.
