Job summary

AccentCare in Florida is seeking a Hospice Admissions RN for full-time coverage in Pinellas County. The role involves assessing patients, coordinating with the Interdisciplinary Group, and ensuring care plans meet needs.

You will participate in on-call or admissions rotations and provide caregiver education as part of the care plan. Qualified candidates are licensed RNs with an associate degree preferred, CPR recommended, and willingness to work with interdisciplinary teams across the hospice

Qualifications

• Licensed RN in state(s) of practice with an associate degree preferred and CPR certification recommended.
• Must meet state-specific regulatory requirements for direct patient care.
• Experience in hospice, home health, medical-surgical, or population-specific care preferred.

Responsibilities

• Assess patient and family needs and collaborate to establish the plan of care.
• Coordinate with physician, IDG, and agencies to optimize care and resources.
• Maintain and document patient records according to protocol.
• Participate in on-call or admissions rotation as directed.
• Provide education to patient and caregiver based on needs and plan of care.
