Job summary

Gilchrist Hospice Care seeks a Hospice Liaison Nurse to generate admissions from hospital referrals and coordinate clinical assessments for prospective patients. The role involves cultivating relationships with hospitals, physicians, social workers, and other referral sources to meet service area goals.

The ideal candidate will have an RN license in Maryland with 3+ years in hospice, home health, critical care, ER, oncology, or related fields, and strong communication and presentation skills.

Qualifications

• Active Maryland RN license and current licensure
• Minimum 3 years hospice/home health/critical care/ER experience
• Strong communication, negotiation and presentation skills
• Ability to travel within Gilchrist service area and manage multiple relationships

Responsibilities

• Generate and facilitate patient admissions from hospital referrals.
• Develop and nurture external professional relationships with hospitals and referral sources.
• Coordinate admissions and discharge planning in collaboration with hospital staff and hospice physicians.
• Represent Gilchrist Hospice Care professionally to the medical community and provide education on services.
