Job summary

GBMC HealthCare is seeking a Hospital Liaison Nurse to generate admissions from hospital referrals, coordinate clinical assessments, and build relationships with hospitals in the Baltimore area. You will travel to patient locations and work with hospital teams to ensure smooth care transitions and appropriate admissions.

The role emphasizes professional communication, strategic outreach, and adherence to regulatory requirements while delivering compassionate, patient-centered end-of-life care

Qualifications

• Registered Nurse licensed in Maryland with current credentials.
• Minimum of three years experience in hospice, home health, critical care, ER, oncology, or related outreach.
• Excellent communication, negotiation, and public relations skills.

Responsibilities

• Actively develops and fosters hospital referral source acceptance for GHC.
• Increases admissions from referral sources and maintains relationships with hospitals.
• Performs or coordinates clinical assessments and ensures HIPAA/compliance standards.
• Delivers presentations and educates the medical community on end-of-life services.
