Job summary

Compassus is seeking a Hospice Admission Registered Nurse (RN) to assess patients for hospice eligibility, coordinate at‑home care, and support families during end‑of‑life care. You will collaborate with an interdisciplinary team to develop initial plans of care and ensure patients' needs are addressed promptly.

The role requires an active RN license in Maine, at least two years of RN experience (hospice preferred), and a valid driver's license.

Qualifications

• Two years of RN experience, hospice experience preferred.
• Current CPR and BCLS certification.
• Current driver’s license.

Responsibilities

• Enable patients to spend quality time with their loved ones and doing the activities they enjoy.
• Assess patients for hospice eligibility and explain hospice care services to patients, families/caregivers.
• Ensure all patient’s initial needs are addressed.
• Collaborate with the Inter-Disciplinary Team members regarding initial plan of care, patient’s condition, and other essential information

Skills

Two years RN experience
