Job summary

A Plus Hire Right Search Group is seeking a Hospice Admissions Registered Nurse in Columbus, OH. This role serves as the initial clinical contact for prospective hospice patients and families, handling eligibility assessment, education, admission processing, and care planning.

You will collaborate with physicians and the interdisciplinary team to ensure timely, compassionate admissions and coordinate medications, equipment, and services.

Qualifications

• Current, unrestricted Ohio RN license.
• Minimum of one year in hospice nursing.
• Knowledge of hospice eligibility and admission processes.
• Valid driver's license.
• Willingness to travel throughout the territory.
• On-call availability as needed.

Responsibilities

• Evaluate prospective patients to determine hospice eligibility.
• Review diagnoses, medical history, symptoms, and functional status.
• Educate patients and families on hospice services and Medicare benefits.
• Complete admission assessments, consents, and documentation.
• Develop individualized plans of care and manage admissions.
• Coordinate meds, equipment, and services for smooth transitions.
• Collaborate with interdisciplinary team for admissions.
