Zusammenfassung

Arkansas Hospice is seeking an Access Registered Nurse (RN) in Batesville, AR to coordinate inquiries, referrals and admissions for hospice care across home base and in-patient facilities.

The RN will interface with discharge planners, physicians and staff, assist in intake, and may provide care for General Inpatient (GIP) patients in contracted hospitals. A year of RN experience and AR licensure are required.

Qualifikationen

• Graduation from an accredited nursing school.
• Minimum of one year RN experience in a hospital, home health, hospice, or related program.
• AR RN licensure must be active and in good standing.

Aufgaben

• Greets customers on the telephone and records patient information and referrals.
• Assists in admission processes and coordinates transfer of information to teams.
• Evaluates patient eligibility and level of care for hospice services.
• Completes admission documentation in the electronic health record.
• Acts as patient/family advocate and coordinates care with interdisciplinary team.
• Participates in QAPI program and maintains confidentiality.
