Avow Hospice in Naples, FL is seeking a Patient Access Specialist to coordinate referrals from hospitals and physicians. You will interface with families, schedule admissions, and enter data into the EMR to support seamless transitions into hospice care.

The role requires strong customer service, basic medical terminology knowledge, and bilingual Spanish preferred. The position includes comprehensive benefits and an emphasis on team collaboration.

Qualifications

• Associates Degree; or one to two years related experience and/or training.
• Experience in a customer-facing role in healthcare preferred.
• Knowledge of basic medical terminology.

Responsibilities

• Act as primary in-office contact in Patient Access Center via telephone for referrals.
• Respond promptly to referral inquiries with professional service.
• Develop and maintain relationships with physician offices and hospitals.
• Contact patient and family soon after referral is received.
• Schedule admissions center appointments for same day or ASAP.
• Enter demographic and billing information into EMR for inquiries, referrals, admissions.
• Coordinate with management to facilitate referral-to-admission workflow.
• Provide timely information on Avow hospice services and benefits.
