Job summary

Avow Hospice in Fort Myers, FL is seeking an experienced Patient Access Nurse to coordinate referrals and admissions, ensuring smooth transitions to hospice services. The role requires excellent communication with physicians, facilities, patients, and families, and involves documenting in the EMR and guiding insurance information through billing.

The position includes weekend rotation and a $15,000 sign-on bonus for qualified candidates.

Qualifications

• Associate degree in nursing is required.
• Florida RN license with CPR; CHPN preferred within two years of hire.
• Bachelor’s degree preferred for advanced responsibilities.

Responsibilities

• Coordinate and complete referral and admission processes for smooth hospice transitions.
• Maintain communications with physicians, hospitals, and referral sources.
• Document in EMR and assist with billing information during admission.
• Provide education and emotional support to patients and families.
• Coordinate discharges and ensure supplies and equipment are arranged as needed.
• Assist with a teaching plan based on patient/family needs.
