Qualifications

• Associate degree required; Bachelor’s degree preferred; RN license (Florida) required.
• CPR certification; CHPN preferred, within 2 years of hire or 1 year after transfer.
• Strong computer skills; able to document in EMR; valid driver's license.

Responsibilities

• Coordinate referral and admission process for smooth transition to hospice services.
• Meet with physicians and facilities to maintain open communications.
• Provide information on Avow services and hospice benefits; collect necessary documentation.
