Qualifications

• Minimum of three (3) years training or equivalent work experience in clerical, administrative or billing services preferred.
• Must possess a minimum of intermediate computer and software skills.
• Netsmart EMR knowledge preferred.
• This position also requires strong communication skills including excellent verbal and writing abilities.
• This position also requires an understanding of end of life care, and must have the ability to maintain harmonious, constructive relationships with internal and external customers.
• Attendance is a condition of employment.

Responsibilities

• Respond to referrals from physicians, hospitals, SNFs, ALRs and the community with exceptional customer service.
• Perform data entry and retrieve information from various software systems, documenting, processing and following up on hospice referrals.
• Serve as a customer service liaison with referral sources to obtain documentation to support terminal diagnosis and admission to hospice.
• Coordinate with community and facilities to meet their service needs.
• Demonstrate effective written and verbal communication skills; provide information to prospective patients, families, and referral sources.
