Job summary

Peoples Health Services in the Florida Panhandle seeks an Admissions Nurse to perform hospice consultations and admission visits, assessing patient and family needs to determine eligibility for hospice care. You will collaborate with attending physicians and complete admission documentation for Hospice Election of Benefit.

The role requires an RN license, CPR certification, and flexibility for after-hours and weekend work to support compassionate patient care.

Qualifications

• Active Florida or multi-state RN license.
• Two years of RN experience preferred.
• Experience in hospice, home health, oncology preferred.
• Availability for evenings, nights, weekends and holidays.
• Current CPR certification is required.
• Valid driver’s license and auto insurance.

Responsibilities

• Performs hospice consultation and admission visits to assess patient/family needs and determine eligibility.
• Completes admission assessment and Hospice Election of Benefit documentation.
• Coordinates with attending physicians to ensure smooth admission and care planning.
• Maintains accurate records and participates in after-hours on-call rotation.
