Aspire Indiana Health seeks a HOPE Case Manager to empower individuals through intake assessments, personalized care planning, referrals, and coordinated discharge planning across multiple counties.

The role requires a Bachelor's in a human services field and at least 1 year in case management; knowledge of HIV/Hepatitis C and Indiana Medicaid is highly preferred. Evening/weekend hours may be required.

Qualifikationen

• Bachelor’s Degree in a Human Services related field from an accredited college/university.
• One year of experience in case management or approved funding-source work experience.
• Must have knowledge of community resources to best serve clients.
• Basic knowledge of HIV, Hepatitis C, and Harm Reduction Principles preferred.
• Knowledge of Indiana Medicaid, HIP 2.0, and Marketplace coverage preferred.

• Conduct intake assessments and develop personalized care plans.
• Make referrals and re-evaluate client progress.
• Coordinate discharge planning for HOPE services.
• Support program implementation, evaluation, and funder compliance.
• Build partnerships with key stakeholders to enhance community impact.
