Job summary

Aspire Indiana Health in Indiana is interviewing for a HOPE Case Manager to empower individuals toward optimal health through intake assessments, personalized care plans, referrals, and discharge coordination. The role requires a Bachelor's in Human Services, at least one year of case management, and knowledge of community resources; HIV/HCV knowledge and Medicaid HIP 2.0 familiarity are preferred.

Some evening/weekend work may be required.

Qualifications

• Bachelor’s degree in a human services field required.
• At least 1 year of case management or related experience.
• Knowledge of community resources to best serve clients.
• HIV/Hepatitis C knowledge and harm reduction principles preferred.
• Indiana Medicaid/HIP 2.0/Marketplace knowledge preferred.

Responsibilities

• Conduct intake assessments and develop personalized care plans.
• Make referrals and coordinate discharge planning for HOPE services.
• Monitor progress and re-evaluate plans with clients.
• Ensure program implementation, evaluation, and funder compliance.
• Build partnerships with key community stakeholders to expand impact.
