Job summary

Aspire Indiana Health is hiring a HOPE Case Manager to empower individuals through intake assessments, care plans, referrals, and discharge coordination across multiple counties. This role uses strengths-based, client-centered approaches to help clients access resources and navigate barriers to care.

Candidates should have a Bachelor's in Human Services and at least one year of case management experience, with knowledge of community resources and Indiana Medicaid; willingness to work evenings

Qualifications

• Bachelor’s Degree in a Human Services field or HIV-related work experience may be considered.
• One year of case management, client management, or approved work experience required.
• Knowledge of Indiana Medicaid, HIP 2.0, and Marketplace coverage highly preferred.

Responsibilities

• Conduct intake assessments and develop personalized care plans.
• Make referrals, reevaluate progress, and coordinate discharge planning for HOPE services.
• Support program implementation, evaluation and funder compliance, and build partnerships with key stakeholders.
