Job summary

Kahu Malama Nurses in Honolulu, HI seeks a Healthcare Admin to support home health referrals for Medicare-certified services. You will work with physicians, clinics, and clinical staff to ensure smooth patient transitions to home health care.

Responsibilities

• Coordinate with physicians' offices, hospitals, clinics, and other referral sources.
• Review and organize referral documentation.
• Communicate with internal departments and external healthcare providers to facilitate continuity of patient care.
• Provide administrative support for office and clinic referrals as needed.
• Receive, review, and process patient referrals.
• Answer phone calls and communicate professionally with patients, providers, and healthcare facilities.
