Job summary

Qualifications

• High school diploma or GED.
• Driver's License required at hire.
• Basic Life Support required at hire.

Responsibilities

• Promote KP Hospice services and coordinate care transitions.
• Develop access to KPCAH Hospice within service areas.
• Build relationships with KP staff, patients, families, and community partners.
