Job summary

The City of Miami is seeking a dedicated outreach and referral coordinator to support homeless outreach operations. You will coordinate referrals and placements, engage clients, and help ensure accurate documentation and service coordination across field and office settings.

Responsibilities

• Coordinate referral and placement workflow activities for the homeless outreach program.
• Screen and assess individuals for placement eligibility and connect to beds.
• Maintain daily shelter and program bed inventories and allocate beds appropriately.
• Ensure accurate documentation and timely reporting of service coordination activities.
• Provide information on placement procedures, eligibility, and available resources.
• Enter client data into HMIS and related systems per policy.
• Prepare referral documents, placement records, and operational reports.
• Resolve service coordination and placement issues arising during field and office operations.
