Job summary

Catholic Charities Brooklyn and Queens, Inc. seeks a Case Manager II to provide comprehensive support to clients at risk of homelessness.

You will conduct assessments, intake, crisis intervention, information and referral, and long-term case management to connect participants with housing and financial assistance. Responsibilities include regular client visits, maintaining case records, inputting data, and coordinating referrals.

Qualifications

• BS degree in Human Services or related field.
• One year of experience in social services or direct client work.
• Bilingual skills in Spanish are a plus.
• Flexible hours, including weekends/evenings.
• Able to conduct home/field visits and travel across the five boroughs as needed.
• Lifts and/or moves up to 10 pounds.

Responsibilities

• Provide case management, intake, and eligibility assessment.
• Maintain regular visits and contact with participants.
• Establish and maintain case records and input data.
• Assess basic needs including food, clothing, shelter.
• Follow-up on referrals and coordinate services.
