Catholic Charities Brooklyn and Queens, Inc. is seeking a dedicated Case Manager II to provide critical support services to clients at risk of homelessness.

Under the supervisor's direction, you'll render professional intervention, conducting assessments and crisis interventions while ensuring comprehensive case management. This role requires a BS degree in Human Services or a related field, preferably with one year of experience in social services.

Qualifications

• One year experience in a social service setting.
• Ability to work flexible hours including weekends and evenings.
• Able to conduct home visits and travel throughout the five boroughs.

Responsibilities

• Provide case management services and assess client eligibility.
• Conduct regular visits for assigned cases and maintain case records.
• Assess basic needs such as food, clothing, and shelter.
